Sudden unexpected death in epilepsy: Who are the children at risk?

چکیده
The Canadian Paediatric Surveillance Program (CPSP) is a joint project of the Canadian Paediatric Society and the Public Health Agency of Canada, which undertakes the surveillance of rare diseases and conditions in children and youth. For more information, visit our website at www.cpsp.cps.ca. CASE 1 A developmentally appropriate 14-year-old girl presented with a history of two generalized tonic-clonic seizures. Further history was positive for myoclonic jerks, most often in the morning. An electroencephalogram demonstrated generalized spike and slow wave discharges, and a diagnosis of juvenile myoclonic epilepsy was established. She was initially treated with valproic acid, which resulted in unwanted side effects and was changed to lamotrigine. During the period of medication changes, she experienced another generalized tonic-clonic seizure. Over the next two years, she had two additional generalized tonic-clonic seizures. With questioning, these seizures could be attributed to forgotten medication doses. She was an excellent student and excelled in athletics. Three years after her initial diagnosis, at 17 years of age, she was found one morning, dead in her bed in the supine position. There was evidence that a seizure had occurred, including tongue lacerations and disruption of the bed sheets. Autopsy did not reveal the cause of death. As expected, the family was profoundly distraught, heightened by their anger at never having been told of the risk of death associated with epilepsy. In follow-up discussions with her parents, they expressed concerns about their daughter’s compliance with anticonvulsant medications.

Structural imaging biomarkers of sudden unexpected death in epilepsy
Sudden unexpected death in epilepsy is a major cause of premature death in people with epilepsy. We aimed to assess whether structural changes potentially attributable to sudden death pathogenesis were present on magnetic resonance imaging in people who subsequently died of sudden unexpected death in epilepsy. In a retrospective, voxel-based analysis of T1 volume scans, we compared grey matter ...
متن کاملExplaining the unexplained; expecting the unexpected: where are we with sudden unexpected death in epilepsy?
Sudden death is over 20 times more frequent in people with epilepsy than the general population. The literature on clinical risk factors is now able to define individuals at the highest risk. Despite these advances in our understanding of risk, the mechanism of sudden unexpected death in epilepsy remains elusive. While it is unlikely that a single mechanism will be found to explain all deaths, ...
متن کاملDeath in children with epilepsy: a population-based study.
BACKGROUND Families of children with newly diagnosed epilepsy worry about death during a seizure. We aimed to assess the frequency and causes of death of children with epilepsy. METHOD We did a population-based cohort study. The Nova Scotia epilepsy cohort includes all children who developed epilepsy during 1977-85. In 1999, we matched names and birth dates with provincial health-care, death,...
متن کاملSudden unexpected death in epilepsy: an important concern
Epilepsy is one of the most common neurologic problems worldwide. Unfortunately, individuals with epilepsy are at higher risk of death than the general population, and sudden unexpected death in epilepsy is the most important direct epilepsy-related cause of death. In this review article, our research group focused on the risk factors, mechanisms and preventative measures obtained from clinical...
متن کاملA Brief Overview of Epilepsy with Emphasis on Children
Epilepsy is a brain disorder in which a person has repeated seizures over time. A seizure is a sudden change in the electrical and chemical activity in the brain. A single seizure that does not happen again is NOT epilepsy. Most children with epilepsy live a normal life. Certain types of childhood epilepsy go away or improve with age, usually in the late teens or 20s. Nearly 80% of people with ...
متن کاملSudden unexpected death in epilepsy: neuropathologic findings.
Sudden unexpected death in epilepsy refers to sudden death of an individual with a clinical history of epilepsy, in whom a postmortem examination fails to uncover a gross anatomic, toxicologic, or environmental cause of death. Evidence of terminal seizure activity may not be present. One to two percent of natural deaths certified by the medicolegal death investigator are attributed to epilepsy....
